Introduction: Pituitary Adenomas account for approximately 10-15% of all intracranial tumors. They are classified according to their secretory activity, presenting with varied hormonal and visual symptoms. The aim treatment pituitary adenomas is reversal endocrine dysfunction preservation normal function, along decompression nervous structures control tumor growth in large Aims Objective: this study was report the efficacy safety Trans- Sphenoidal surgery. Material Methods: 30 cases were included study.All patients assessed clinically symptoms, signs headache others. Clinical radioimaging preoperative assessment done patients. Reassessment accordingly postoperative period follow up examination. Immediate outcome (course), complications, radiotherapy or medical therapy resultswere assessed. Results: Most common age group 41-50 years, male:female incidence 1:1.3. Headache followed by Visual disturbance most clinical symptoms observed. Commonest field defect observed bitemporal hemianopia. Increased GH level (33%) hyperprolactinemia (13%) endocrinologic abnormalities. Post operative complications CSF leak Diabetes insipidus
